Works with participants (or caregiver) to develop a long-term care (LTC) plan and assists the participants to manage their care needs. Duties Include: The RNSC will assess the functional, psychological, financial, and environmental needs of potential Care Management participants and participants in the Veteran Directed (VA) program. Work with the participant (and his/her selected designee) to develop a person-centered plan; assist and support the participant to manage their care needs. Provide LTC options and strategies including the eligibility, frequency, duration, and financing of services. Educate participants/caregivers regarding the participant's health status, health promotion, and risks and benefits of treatment options.
