POSITION SUMMARY: To provide collaborative, evidence-based patient care for the purpose of achieving improved medication-related therapeutic outcomes and continuity of care. To be the medication and pharmacy practice expert serving as a resource within the pharmacy department, inter-disciplinary teams and throughout the organization and share this expertise with the team by teaching and mentoring pharmacy learners, pharmacy team members and interprofessional team colleagues. Working collaboratively within a multidisciplinary health care organization, the Pharmacotherapy Specialist provides medication use, clinical and pharmacy practice expertise as an integral member of the Pharmacy department and organization. The Pharmacotherapy Specialist provides expertise in delivering pharmaceutical care to patients, including verifying medication orders, assessing patients’ medication therapy and identifying and resolving actual and potential Drug Therapy Problems (DTPs).
