Job description

Position Summary Responsible for the implementation of the Case Management process. Monitors physician documentation and works closely with the medical staff for appropriateness of hospitalization. Identifies and evaluates quality of care, infection control, safety hazards, potential risk management and utilization problems. Reports findings to Director for dissemination to appropriate department and/or committees. Responsible for initial utilization screen and ongoing utilization review through discharge. Identifies discharge and transition needs and is ultimately responsible for the coordination of discharge planning activities. Works collaboratively with interdisciplinary staff internal and external to the organization. Participates in quality improvement and evaluation process. Primary duty of performing office or non-manual work directly related to the management or general business operations of the employer or employer’s customers. Primary duty includes the exercise of discretion and independent judgment regarding matters of significance. Education: Graduate of an approved school of nursing.
