Inspire health. Serve with compassion. Be the difference. Job Summary: Provides critical psycho-social support to Palliative Care patients and their families through the Palliative Care consult team. Utilizes professional background and skills to assist the team and facilitate patient-family meetings. Assist team, attending physicians, staff, and patient and family in the clarification of the goals of care and the plan of care. Essential Functions: All team members are expected to be knowledgeable and compliant with Prisma Health's purpose: Inspire health. Serve with compassion. Be the difference Participates in the Multidisciplinary Team’s initial assessments, care planning and periodic re-assessments. Assessments are to be completed prior to the scheduled team meetings. Communicates patient changes to team members. Maintains current written case management records both in paper and electronic format as required, including periodic reassessments of program participants. Provides individual and family counseling, develops and leads group counseling and activities.
