Department: 37613 Rehabilitation Services: IL - Outpatient Rehabilitation: AMG 12/120 Status: Part time Benefits Eligible: Yes Hou rs Per Week: 20 Schedule Details/Additional Information: Business hours: Monday-Friday with potential for Saturday hours in the future pending access needs. OFFERING A $2500.00 SIGN-ON BONUS PAYABLE IN TWO INSTALLMENTS - 50% AFTER 90 DAYS - 50% AFTER ONE YEAR Pay Range: $38.20 - $57.30 Major Responsibilities: Interprets physician referrals and evaluates the patient's physical, perceptual, cognitive, social, and vocational abilities and their impact on the patient's activities of daily living. Establishes a plan of care and implements individual treatment based on evaluation results, functional goals and by utilizing appropriate techniques. Provides and/or fabricates assistive devices and/or splints to enhance patient's functional independence. Monitors and evaluates outcomes and appropriately adjusts individualized treatment plan based on patient's response and provides status updates to physicians and other teammates.
