Job Overview: The Coordinator, Community Dental Home (CDHC) is responsible for increas ing access to dental care for children, perinatal patients, and adults. The CDHC duties include, but are not limited to, oversight (parental/family education), dental home referral coordination, evaluation , training, onsite face-to-face with patient (PSA), transport portable dental units, and mobile driving as needed.
