RN Case Manager | Hospice

Find your purpose with CentraCare by joining our Hospice team as an RN Case Manager! This position will serve patients primarily in St. Cloud, Sauk Rapids, Royalton, Little Falls and surrounding areas. You will make a difference in the lives of others by caring for patients and their families in the comfort of wherever they call home. Responsibilities include completing medication reviews, seeking orders from physicians, and teaching caregivers about how to provide end of life care for their loved ones. Other routine nursing tasks include wound care, catheter care, head to toe assessments, and pain assessments while tending to ongoing case management with the team. We provide an extensive orientation program as well as ongoing RN office support to you while visiting patients. Schedule: - Full-time | 72 hours every two weeks - Monday - Friday | Day shift typically 8:00 am – 5:00 pm - Every 4th weekend working rotating day and evening shifts - Two rotating holidays annually | Rare on-call shifts Pay and Benefits: - Pay based on experience and degree - Associate/Diploma in nursing starting at $38.21 per hour - Bachelor’s in nursing starting at $39.16 per hour - Master’s in nursing sta...
