This Position Starting Min Pay is $27.15 Job Summary: Summary Responsible for overseeing and coordinating community health programs and activities, as well as providing leadership, training and guidance to Community Health staff. Areas could include exercise, mental health, medication management, nutrition, health care system navigation, substance use and other health behaviors. They work closely with healthcare providers to ensure that patients receive appropriate care and support services to improve health outcomes.
