We’re rebuilding how healthcare works by bringing primary care into non-primary care settings and redefining the role specialists can play in driving patient outcomes. Triage is temporary. Treatment is transformative. Our care teams provide wrap-around care for patients outside of their usual specialist appointments. This care team includes physicians, advanced practice providers, care navigators, pharmacists, dietitians, and social workers and works to solve both the immediate challenges and the long-term trajectory of every patient we touch. We put patients at the center of value-based care and design everything else around that. We deliver whole-person care, wherever the patient is – in clinic, through telehealth, at home.
