Job Summary: Collaborates with patient, family, physicians, case managers, health care team, and community resources to develop a plan which addresses the patient's post-hospital medical needs. Assists and provides patient care coordination and referral services for patients discharged from an in-patient setting or other outside sources and refer them to the home health agency. Works in conjunction with hospital case management and coordinating discharge planning for home care services.
