Nurse Practitioner 1 FTE: Thunder Bay Site One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services. We are currently seeking a 1.0 FTE Nurse Practitioner to join our team of highly skilled nurses, system navigators, social worker, dietitian, nutrition worker and recreation therapist in a team model, supporting the delivery of services through our Community Wellness Hubs. Community Wellness Hubs provide barrier free access and attachment for clinical care, illness prevention, and health promotion services in community settings while fostering strong partnerships across the Thunder Bay service area. The Nurse Practitioner will provide comprehensive primary health care services with a strong emphasis on disease prevention, health promotion, client education, and empowerment.
