It’s the way you want to be treated. Position Summary: Monitors, coordinates and evaluates the care and appropriateness of hospitalization; initiates referrals. Participates in development, implementation and analysis of care delivery standards and outcome measures. Essential Duties and Responsibilities: - Completes Continuity of Care assessments and establishes appropriate plans by initiation of timely interventions through collaboration with patients, families and multidisciplinary care team respecting choice of patient - Concurrently reviews clinical documentation in medical record to assure timely progression of care transitions - Collaborates with multi-disciplinary team and informs them of variance from established care pathways to ensure timely and appropriate progression of care and eliminates barriers for next site of care transitions - Assists in expediting care delivery to avoid delays in timely service provision - Provides feedback to medical s...
