Our communal goal is to attach, ensure access and provide comprehensive medical care to patients living in the local community.We have exciting opportunities for medical staff and health care professionals to join our Family Health Clinic team. Be part of a team-based model, working together with physicians, nurse practitioners, registered nurses, social workers, dietitians, and medical office assistants, pharmacists, community support liaison workers, practice coaches to provide quality patient care. JOB SUMMARY: Within the context of a team-based model of care and in collaboration with the Primary Care Provider (Family Physician or Nurse Practitioner) and other members of the PCN Interdisciplinary team, the Primary Care Dietitian provides nutrition care through health promotion, disease prevention, treatment, support and rehabilitation to enable clients to achieve and maintain optimal health outcomes.
