Overview About the Role: The primary purpose of the Senior Community Health Worker will play a pivotal role in coordinating and overseeing Community Health initiatives to promote the overall health and well-being of our University Hospital patients. This position requires a strong commitment to community engagement, program coordination, and collaboration with various stakeholders. The Senior CHW is an integral member of the multi-disciplinary team that works to endorse clinical teams to the community. Provides perspectives to enhance care management strategies while efficiently navigating community resources to provide patients with access and education on the importance of preventive care screenings and services.
