Position Title Discharge Nurse Days - Full Time: Liberty Hospital: Position Summary / Career Interest: 0900-1700 M-F no weekends, no holidays Responsible for the delivery of professional nursing services which will address the physical, psycho-social, spiritual, cultural, developmental, and age-appropriate needs of each individual patient/family. Provides care with a focus on discharge preparation and instruction, and coordinates discharge planning with other disciplines. Responsibilities and Essential Job Functions: - Performs patient assessment - Utilizes plan of care - Validates that patients have clear and concise information and understanding of discharge instructions - Completes accurate discharge medication reconciliation - Assesses patient/family learning needs - Utilizes interdepartmental collaboration according to patient needs to prepare for discharge - Provides staff education on discharge process - Ensures safety of patients, visitors, colleagues and self - May perform Staff Nurse duties as needed - Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department. - These statements are intended to describe the essenti...
