Franciscan Health Lafayette East Campus 1701 S Creasy Ln Lafayette, Indiana 47905 The Birth Registrar at Franciscan Health conducts phone and bedside interviews with parents of newborns in a professional manner, to obtain accurate demographic information. The position files birth certificates with the state Bureau of Vital Statistics within five days of birth via the TER web-based software.
