Hours/Shift: Full Time/Rotating: Position Purpose: The DMC encompasses a team of pharmacists providing disease state medication management services through a Collaborative Practice Agreement with our community practitioners (Primary Care, Endocrinology, Oncology, Cardiology, Hepatology, and more). This is a setting where a multidisciplinary health care team approach is valued and expected. Working at the top of a pharmacist’s scope of practice, the specialist will manage a variety of disease states through delegated prescriptive and lab ordering authority.
