Description Jet Health, Inc./Klarus Home Health is seeking a full time LVN to join our team as an Intake Coordinator in our Fort Worth, TX office. The Intake Coordinator will be responsible for the timely coordination of the referral process including the review/response/coordination of care for patients referred to Klarus Home Health/Jet Health, Inc. Qualifications: - Coordinating the referral process for patients referred to Klarus Home Health/Jet Health, Inc. - Monitoring, reviewing, and responding to patient referrals. - Developing referral relationships within the community. - Interacting with facility representatives, discharge planners, and other agencies to facilitate care initiation. - Conducting reverse marketing in coordination with the Business Development Leader. - Education: High School graduate or equivalent and successful completion of LVN program - Licensure: Current state license as a Licensed Vocational/Practical Nurse or a multi-state license under the National Nurse Licensure Compact, required - Skills: Compassionate nature, ability to work independently, good communication, reliability, and organizational skills.
