WHAT YOU WILL DO: The Elder Services Case Manager is a non-exempt position responsible for caseload management within the elder population, which may include Adult Protective Services High Risk referral. This position coordinates services, develops care plans, and implements and monitors service delivery. - Provide case management services in accordance with guidelines and requirements as established by the funding source - Screen, assess, and evaluate clients for service needs and develop and implement care plans to address service needs - Evaluate client eligibility for programs, including assistance with Medicaid eligibility - Maintain regular contact with clients as established in program guidelines - Provide information, referral, and follow-up for appropriate support services, including coordination of client services with Medicare or Hospice agencies - Resolve service delivery problems - Complete and submit accurate and timely documentation, in...
