Job DetailsJob Location: Simpsonville (Upstate) - Simpsonville, SC 29680Position Type: Per DiemAbout the Role Providence Care is seeking a compassionate and dedicated Hospice Chaplain & Bereavement Coordinator to provide spiritual support and bereavement care to our patients, families, and caregivers. This role ensures that spiritual and bereavement services align with federal, state, and local regulations while serving as an integral part of the interdisciplinary hospice care team. Key Responsibilities: Chaplain Functions: Assess patients' and families' spiritual needs within five days of admission Develop and update a spiritual care plan in alignment with the hospice care plan Provide pastoral care, counseling, and religious sacraments as appropriate Collaborate with the interdisciplinary team to ensure holistic patient care Build relationships with community clergy to facilitate additional chaplaincy support Offer individual, family, or group counseling as needed Participate in community education initiatives about hospice care Bereavement Coordinator Function: Lead and manage the bereavement services program Develop and oversee grief support programs for patients' loved ones Co...
