Position Overview: The Case Manager works closely with a multidisciplinary treatment team—including clinical staff, admissions, nursing, and business development—to facilitate patient recovery. This role ensures patients have thorough aftercare plans, addresses individual needs, and supports patients in achieving the best possible outcomes. Key Responsibilities: - Complete patient admission assessments and document in Avatar within 72 hours of admission. - Collaborate with patients and support systems to create and implement individual Continued Care Plans (CCP). - Address patient needs across life domains: housing, employment, legal, financial, family, health, relapse prevention, and more. - Schedule post-discharge appointments, including SUD/MAT and primary care...
