JOB SUMMARY: Under the supervision of the Clinical Director, the Clinician is responsible for ensuring that proper clinical and case management services as well as community reintegration planning are provided to the members. This role will have a large outreach component, as this person will act as a liaison between members and community connections, as they begin their process of transitioning to a more independent status within this community. PROGRAM/DEPARTMENT RESPONSIBILITIES: · Ensure that an intake screening (suicide/safety assessment) is completed for each new intake to the program. · Provide weekly individual therapy to members and complete required Department of Youth Service (DYS) clinical progress notes. · Provide group therapy several times a week. · Provide family therapy as needed. · Develop an individual service plan for each member and create monthly treatment plan reviews with individualized goals and progress reports. · Schedule and facilitate member treatment meetings (monthly) to review member progress.
