Key Tasks and Responsibilities: - Serve as a primary contact for patients and families by phone, addressing concerns, triaging symptoms, and escalating issues to providers as appropriate. - Coordinate prescription refill requests, ensuring accuracy, compliance with pain management policies, and timely provider review/authorization. - Monitor and track prior authorizations, referrals, and diagnostic orders related to pain management. - Provide patient education regarding medication regimens, treatment plans, and follow-up requirements. - Conduct proactive follow-up calls to assess pain control, adherence to treatment, side effects, and patient satisfaction. - Collaborate with providers and the care team to support individualized pain management plans. - Document all patient interactions and interventions in the electronic medical record (EMR) with accuracy and timeliness. - Support care coordination between the Pain Center, primary care providers, pharmacies, specialists, and ancillary services (e.g., therapy, imaging). - Conduct occasional in-office patient visits, including vital signs, medication reconciliation, post procedure follow-up visits and basic nursing assessments. - Ass...
