Guthrie Home Health is seeking a Full-Time Admissions RN to support our Cortland, Tompkins, and Northern Tioga County service areas, with flexibility to support our broader five ‑ county region based on patient needs. As an Admissions RN, you'll perform comprehensive patient assessments in the home setting, establish care plans that guide the full episode of care, and serve as the first point of connection for patients entering Home Health services. You’ll build deep relationships with patients and families, providing education and support that empowers them to return to independent health management.
