PAY: Salary will be commensurate with experience and qualifications, ranging from $36,317.40 to $42,487.32, annually. DUTIES AND RESPONSIBILITIES: Duties and Responsibilities include but are not limited to the following: - Coordinates and conducts comprehensive assessments of applicants for nursing facility placement and home and community-based services.
