Be the difference Provides competent service in the medication use process as related to prescribing, administration and monitoring with an overall emphasis on the provision of most efficacious, safe and cost-effective therapy. This will be accomplished through the provision of accurate up to date pharmacotherapy management to all patient populations serviced. Activities include, but are not limited to, participation in patient care rounds, individual patient care assessment (patient specific problem list, medication profile and administration record review), development of individual pharmacotherapy plans (selection of drug therapy, monitoring plans) and clinical consults and committee work.
