Job DetailsJob Location: Columbus, OH 43224Position Type: Full TimeSalary Range: $20.00 HourlySummary: Community Health Workers will primarily be in the community assisting patients with the social determinants of health using the Pathways HUB model. The position will assist patients in primary care and behavioral health, but focus primarily on prenatal and post-partum African American women. The position will be required to visit patient homes. CHW's will work closely with medical providers, staff, and other agencies to improve patient care and outcomes.
