Hartfield, Virginia Overview: A Physical Therapist carries out an individualized program for patients with acute and chronic health conditions to maximize their quality of life and achieve their functional goals. The Physical Therapist may work in an inpatient and/or outpatient setting. The Therapist is supervised by Rehab Services management and maintains a positive working relationship with other health professionals and ancillary staff. This position is eligible for a sign on bonus of up to 15,000 for those that qualify! What you will do: - Performs evaluations, determines PT diagnoses, and develops plan of care. Performs physical therapy procedures to aid in restoring function, improving mobility, relieving pain, improving health, and preventing or limiting permanent physical disability. - Evaluates and assesses the patient timely, based on principles outlined by practice guidelines, evidence based guidelines and regulatory standards (state & organizational policies, requirements of accrediting bodies, etc.). - Builds trusting collaborative relationships with a multi-disciplinary team to support patient care needs.
