Behavioral Health Patient Care Associate, Wyman II, Geri Psych Job Description: Primary Responsibilities 1. Provides physical care to assist patients in maintaining nutrition, hygiene, hydration, and elimination. 2. Collects necessary data for the medical monitoring of patients' status (e.g., diagnostic specimens, and weights), and records this accurately. 3. Reports changes in patients' physical condition, e.g., weight loss, sleep, eating patterns, and general comfort of patient. 4. Participates in the evaluation of patients' needs for general and psychiatric nursing care, and carries out treatment plans. 5. Reports changes in patients' mood, affect, behavior, and psychiatric symptoms. 6. Maintains a safe and clean environment for patients. 7. Cares for equipment used in the delivery of care. 8. Participates in milieu groups and activities as needed. 9. Provides one-to-one observation of patients as needed. 10.
