Full-time, Day/Eve 7am-7pm Includes every third weekend & rotating holidays 72 hrs/2 weeks General Summary: Works collaboratively to facilitate a stable and smooth operation of the unit and provides therapeutic interventions in accordance with best practice guidelines, care plans and policies. Duties and Responsibilities: Essential Functions: - Coordinates and monitors the activities of the individual’s plan of care. - Performs all duties and responsibilities of the nursing assistant job description. - Provides direct care to patients primarily assisting with behavioral symptom management techniques and to assist patients in adapting to and/or coping with internal and external stresses. - Provides assistance to the care team in development of and updating the care plan and discharge planning. - Promotes patient attendance and participation in activities and documents participation. - Provides practical help and side-by-side coaching, problems solving, training, and assistance to help patients with their activities of daily living and concerns. - Provides direct care to patients in a manner that considers safety, comfort, and the therapeutic environment. - Assists the team members i...
