Our therapist responsibilities include some of the following: · Psychosocial assessment of new patients; developing an update or detailed reassessment of the patient · Maintains contact with family and significant others in the patient’s life when appropriate and provides patient and family education, support, and advocacy · Provides coordination and acts as liaison with community based social and mental health agencies · Facilitates discharge planning and conducts post discharge follow-up to assure that linkage of the pati...
