Position Summary: - This will be a field-based position working with Aetna members and Community Based Organizations (CBO) in Detroit (West) and Western Wayne county (Canton, Dearborn, Garden City, Inkster, Livonia, Northville, Plymouth, Wayne, Westland). - Community Health Workers (CHW) serve the community as a bridge between the member (community) and the health care system (providers) through face-to-face outreach and education. - By providing these community and home-visiting services, CHW's help Aetna members attain and maintain better health outcomes, improve relationships with healthcare providers and help both parties become more acculturated with each other. - This position will work under the direction of the CHW Supervisor.
