Description Summary: Perform duties associated with admission and retention of new and existing consumers to Intensive In-Home Program. Provide clinical expertise and oversight for the Intensive In-Home Team. Coordinates with Director of Community Based Services. Principal Duties and Responsibilities: Clinical: Administrative: Supervision: Provide Clinical and Administrative oversight to the IIH team members. - Coordinate delivery of time-limited individual and intensive family treatment services intended to stabilize the home environment, promote reunification, or prevent the utilization of out-of-home placement (i.e. psychiatric hospital, therapeutic foster care, residential treatment facility) for the identified youth within designated time frames. - Completes required documentation to enroll and retain consumers in Intensive In-Home Services provided by CHA, including but not limited to: gathering previous treatment histories, development of initial/complete Person-Centered Plan, completion of all paperwork associated with maintaining authorization, and making necessary referrals.
