Position Summary: The Evaluating Clinician works with Medical Director and QHPs in follow up to initial evaluations and assessments; performs subsequent special consultations when indicated by assessment and initial medical evaluation following established protocols. What You'll Do: Essential Job Functions: Ensure that Comprehensive Assessments to establish and/or maintain collaborative relationships with the health provider community, on behalf of the WeCARE Program Perform medical evaluation and assessments which include: 1. Medical history/prior WeCARE history 2. Physical examination 3. Review of prior WeCARE documentation (when applicable) 4. Review clinical documentation provided by the individual (when available) 5. Medical, mental health, substance use, vocational, educational, social history, etc. Perform Sequential Evaluation: Execute best practices, adopt information technology updates, incorporate predictive analytics into patient care flow redesign decisions; and respond to operations issues when they are prioritized with urgency Create participant-specific report(s) listed below after conducting medical evaluations Conduct the sequential evaluation process as defined b...
