Overview Job Summary: Facilitates resource management activities/discharge planning for patients based on identified patients. Coordinates patient’s stay from admission to discharge through collaboration with physicians, nursing staff and other healthcare professionals to enhance continuity of patient care, promote cost effective utilization of resources and enhance patient, nurse and physician satisfaction. Demonstrates through behavior Decatur Morgan Hospital’s mission, vision and values. Responsibilities: Key Responsibilities / Essential Functions: - Proactively identifies or receives referrals for patients who are likely to be in need of discharge planning assistance - Completes initial and ongoing assessment of patients’ discharge needs. - Develops discharge plan based on the assessment and agreed upon goals with the patient, family and interdisciplinary healthcare team. - Contributes to the interdisciplinary plan of care and ensures that the plan is consistent with the discharge plan and patients continuing care needs. - Makes timely referrals to appropriate community services based on identified needs that assure continuity of care through the discharge planning process. - E...
