Our Clinics offer a team-based care model where our physicians lead a multi-disciplinary care team supporting patient’s physical, emotional, and social wellness. The Cardiology Lead specializes in diseases of the heart and blood vessels and is the Cardiology program leader, and part of a clinical care team that focuses on outpatient medicine, continuity of care, health maintenance, and disease prevention for senior patients. The Cardiology Lead manages complex cardiac conditions such as abnormal heartbeat rhythms, and congestive heart failure, as well as provides preventative techniques for dealing with potential heart-related illnesses.
