Job Description: The Special Procedure Patient Navigator - Medical Assistant will perform the duties necessary to coordinate the care of the patients who undergo special procedures such as a colonoscopy, esophagogastroduodenoscopy (EGD) or Stress Test as part of the Special Procedures Program at Salud Family Health Centers. The Special Procedure Patient Navigator approaches his/her tasks in a team-based fashion that supports patients and their families in self-management, self-efficacy and behavior change.
