Job description

Assesses and evaluates patients/families' social, psychosocial and environmental situations to determine patient needs. Demonstrates knowledge of the disease entity and common medical interventions in the service area assigned Requirements: - Licensed Masters Social Work to provide services including discharge planning for acute care population. Inpatient and emergency room assignments. - Experienced with acute inpatient, experience discharges planning acute care. Knowledgeable SNF and home care referrals. Proficient in biopsychosocial assessments. - Participate in an innovative inpatient care program providing leadership to a multidisciplinary team. Geriatric experience. - Apply pertinent state and federal regulations regarding social work and discharge planning, familiarity with post-acute provider services, financial screening, and reporting requirements. - Masters in Social Work from an accredited school required. New York State License (LMSW) required. - Experienced with acute inpatient, experience discharges planning acute care. Knowledgeable SNF and home care referrals.
