Penobscot Community Health Care is seeking an experienced BC/BE Nurse Practitioner or Physician Associate interested in working with the adult and geriatric population as part of our Community Care and Geriatrics (CCG) team. You will provide in-home care as well as direct care and geriatric services in local nursing homes and other long-term care settings in Greater Bangor. CCG is a well-established team of highly skilled physicians, advanced practice providers, licensed clinical social workers, and nurses focused on finding the right health care solutions for patients, caregivers, and their families. Two years’ experience in clinical practice as a Nurse Practitioner or Physician Associate, with background in Community Health, Family Practice or Adult-Gerontology is preferred. - This position is planned full-time, 40 hours per week, generally four 10-hour days. - Direct in-facility care and in-home care. - A high level of autonomy and flexibility is required for this role. - Your schedule will be set at primary facility locations.
