We are seeking a dedicated Patient Care Manager to join our hospice team. Reporting directly to the Executive Director or Administrator, you will oversee patient care activities to ensure the delivery of high-quality, compliant hospice services. This leadership role involves managing clinical operations, coordinating care, and developing staff to uphold the highest standards of patient care and regulatory compliance. - Minimum of 3 years of direct patient care experience, including 2 years in a hospice or home-based care setting - This is an office role (not remote) with some field requirements.
