Job Title Street Outreach Manager Position Summary: The Street Outreach Supervisor leads field-based outreach operations with a dual focus on generating high-quality member referrals into CalAIM Enhanced Care Management (ECM) and Community Supports (CS) and Behavioral Health (BH) and building durable, strategic relationships with community-based organizations (CBOs) across multiple counties and health plans. This role is accountable for outreach performance, referral pipeline development, and the establishment of trusted partnerships that drive sustained referral flow, coordinated care, and improved member outcomes. Salary - $80,000 - $90,000 annually Key Responsibilities: Referral Generation & Pipeline Development: Community-Based Organization (CBO) Partnerships (Strategic Focus) - Lead outreach strategies designed to consistently identify, engage, and refer ECM- and CS-eligible Medi-Cal members - Establish clear expectations for outreach staff related to referral targets, productivity, and conversion outcomes - Monitor referral volume, referral quality, and enrollment conversion rates across counties and health plans - Identify barriers to referral acceptance and implement correc...
