Advanced Care Practitioner - Primary Care Job Summary: This individual will function independently in collaboration with a supervising physician. The emphasis is on routine office visits, minor office-based procedures and follow-up visits with the patient population. Primary Job Responsibilities/Tasks may include, but not limited to: Seeing all patients of Tryon Medical Partners. Performing initial and periodic medical history and physical examinations. Recording findings of examinations, assessments and plan of care. Performing minor office-based medical and/or surgical procedures. Performing telemedicine services. Writing appropriate prescriptions and administering medications. Responding to patient inquires and requests. Ordering and interpreting laboratory and radiology examinations. Determining treatment plan for patient. Documenting all patient encounters, records observations, interventions, and actions in a medically appropriate and timely manner on the patient’s medical record. Consulting with supervising physician as necessary. Working with broader care team to provide services to patients of Tryon Medical Partners. Acts as a patient advocate for the population served.
