About the Role Mater Private Hospital Bundaberg is seeking a highly skilled Clinical Nurse to join our team in a newly created Discharge Coordinator role. The successful applicant will secure Permanent Full Time employment working a 38-hour work week. In this pivotal role, you will lead and coordinate the patient discharge process, ensuring safe, timely, and effective transitions from hospital to home or post-acute care settings. You will work collaboratively within a multidisciplinary team to deliver person-centred care, optimise patient outcomes, and support continuity of care. This is an excellent opportunity for an experienced nurse who is passionate about care coordination, patient advocacy and service improvement. Mater Bundaberg is a 59-bed hospital providing a broad range of surgical, medical and rehabilitation services.
