$4,000 Sign-On Bonus We are looking for a full-time RN Case Manager to join our Home Health and Hospice team! This position pays $42.00 per hour Join our compassionate team as a RN Case Manager! You'll be the heart of hospice care, coordinating care for patients and families during their most important time. What You'll Do: - Manage 10-14 patients, ensuring they receive appropriate care. - Complete assessments, create care plans, and document care. - Collaborate with doctors, social workers, and other team members. - Provide support and education to families. - Enjoy flexible scheduling and work-life balance (avoid burnout!). We're Looking For: - Idaho RN License. - 2+ years of experience (pref. hospice). - Strong communication and teamwork skills. - Valid driver's license & reliable transportation.
