Position Summary: Monitor clients' health and medication needs. These services include documenting medication lists, overseeing medication distribution, and providing medical case management and reminders for medication adherence. Maintain medication logs, monitor vital signs, organize medication storage, and offer health education. Collaborate with community health providers, internal departments, and social services agencies. Coordinate with pharmacies, assist with healthcare appointments, and ensure accurate, real-time documentation of all services to promote continuity of care.
