The hours for this position are 8am-5pm. Provides comprehensive, value-based medication-related care to achieve definite outcomes that improve a patient's quality of life. Acts in collaboration with other healthcare professionals in the shared responsibility for patient medication therapy outcomes. Helps to manage patient chronic medical conditions and medications in between provider visits.
