Employment Type: Part time Shift: Day Shift Description: Per Diem Hospice Registered Nurse (RN) Liaison - The Community Hospice Albany The Community Hospice has a rewarding opportunity for a RN to work as a part of our Hospice Intake RN Team serving as Liaisons in local hospitals (Capital Region). Needs are primarily on weekdays 8-4:30 - as needed Weekday availability for training is a must Position Highlights: National pre-taxed mileage reimbursement Meetings and charting conducted via supplied lap top Comprehensive orientation provided: ​ What you will do: - Evaluates patients for eligibility and interest in hospice services, describes the support and care available from hospice along with the benefits of hospice care. - Coordinates with the patient’s community physician and our hospice medical directors to confirm eligibility for hospice services. - Gathers information and ensures a smooth handoff to the hospice RN admitting the patient/family to services. - Admits to hospice inpatient services upon eligibility, develops a hospice plan of care and collaborates with the hospice interdisciplinary team and the acute care team. - Works closely with the hospital case management depar...
