Job Details Support Services | Kettering| Full Time | Days Responsibilities & Requirements: The Community Health Worker is a neighborhood-based trusted member of the community and serves as a liaison/link/intermediary between health/social services and the community to facilitate access to services. The Community Health Worker also strengthens individual and community agency by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, referrals to community resources, social support, and advocacy. Performs other duties as assigned. Job Responsibilities: This position will provide high quality health navigation and patient education via telephone and in person, for identified patients who meet criteria for the Substance Abuse Mental Health Administration (SAMHSA) grant. Staff will build and maintain internal and external organizational relationships, document interactions, assist with data collection, identify and share community-based resources, and other grant-related duties, as assigned.
