About the Role As a Social Worker, you will play a vital role in delivering strength‑based, person‑centred practice that empowers the people we support to live fulfilled, independent, and self‑directed lives. In this role, you will: Work directly with young people (typically aged 16–25) and their families to undertake holistic, outcome‑focused assessments. Develop strengths‑based support plans that promote wellbeing, independence, and personalised life opportunities. Build effective relationships with partner agencies including education, health, care providers, and community organisations. Provide clear professional analysis and recommendations in line with the Care Act 2014 and related legislation. Manage a varied caseload, ensuring timely responses and high‑quality social work practice. Support young people to access opportunities in employment, education, independent living, and community participation. Promote safeguarding, identify risks, and contribute to the safety and protection of vulnerable young adults.
