Job Title: Licensed Practical Nurse (LPN) - ElderOne (Full-Time, Evenings) Department: Home Care: Location: 2066 Hudson Avenue, Rochester, NY 14617 Hours Per Week: 40: Schedule: Day and Evening Shifts: (Days 7:00am to 3:30pm, Evenings 3:00pm to 11:30pm), every other weekend required. This position has a designated home base facility and may require occasional travel to additional locations. Typically, travel is limited to one to two locations per day, with up to four sites in Rochester, NY and one in Newark, NY. Mileage reimbursement is provided. SUMMARY: The Licensed Practical Nurse is responsible for providing direct patient care, usually under the direction of a Registered Nurse, in accordance with established plans.
